Report LMS issue

When staff found any function doesn't work as expected, please following steps below to report the issue.

Step 2: Filling the form as below

1) Put the issue screenshot into the Photo frame.

2) Note the reproduce steps & other info related to the issue into Description

3) Input the environment info to help IT inspect the issue location in the right panel as:

Program: name of the program that the issue happened.

Date: when the issue happened.

Type of CS: there're 3 option which serve for different purpose;

+ System issue mean that there was some break or functions/features which worked not as expected.

+ Content issue mean that the contents (wording, gramma, ..) display not correct in the application.

+ Request the system improv. you could request for changes that make the application work better.

Account Info to provide the related account that issue happened.

TYPE of OS name, version of the program that happened the issue.

Note: you should screenshot or recording the the short clip that help IT inspect the issue better.

Step 3: report the issue to IT

Email with attachment to IT Campus group (it.campus@alab.edu.vn)

Subject format: [LMS Issue Reporting] Summary of the issue

E.g. [LMS Issue Reporting] Student could not login to the In-class app
